The Hazardous Waste Manifest (RCRA 303)

All RCRA hazardous waste generators, transporters, and treatment, storage, and disposal facilities must use EPA's standardized hazardous waste manifest form. The form is the critical tracking document used by the EPA, DOT, and state agencies to track the proper transport and disposal of hazardous waste. This training program provides specific instructions for completing the form.

NOTE that those who ship hazardous waste are subject to both RCRA and DOT rules and must comply with both EPA and DOT training requirements. Depending on your waste management responsibilities, additional RCRA training may be necessary. Additional DOT training is required if you sign the shipping paper. The person signing the shipper's certification must have personal knowledge of the facts regarding the waste and its proper preparation for transport—

“I hereby declare that the contents of this consignment are fully and accurately described above by the proper shipping name, and are classified, packaged, marked, and labeled/placarded, and are in all respects in proper condition for transport according to applicable international and national governmental regulations."
